Please provide your basic patient profile information to help speed up the registration process. On entry to the clinic you will be asked if you have pre-registered. We will then ask you to provide your name and date of birth to retrieve this profile information. This process DOES NOT register you for the clinic, you will still need to arrive at the Clinic and leave your cell phone number or identify your car so that we can put in you line for services. This speeds of the registration process for you.
We will have all patients wait in their vehicle for services. We will call you or send a person to your vehicle when it is your time to be seen. We look forward to seeing you.
